At the practice of Dr. Sandip Ganguly, we respect your privacy and your right to control your personal data under the Digital Personal Data Protection (DPDP) Act, 2023. Use this form to request the permanent erasure of your personal and health-related information from our systems.

Important Note for Patients:
  • Medical Records: Please note that under Indian Medical Council regulations, doctors are required to maintain certain medical records (prescriptions, clinical notes) for a minimum period (usually 3 years). Deletion requests for legal medical records will be processed in accordance with these statutory retention laws.
  • Marketing Data: Requests to delete data related to website visits, cookies, and lead forms (name/email/phone) will be processed within 30 days.

Request Details

    What happens next?
    1. Acknowledgement: You will receive a confirmation email within 48 hours of your request.
    2. Processing: Our team will review the request against legal retention requirements.
    3. Confirmation: You will receive a final “Data Deletion Certificate” or confirmation email once the process is complete.
